Paper Form Option The paper form is appropriate for those reporting on fewer than 50 engineers and who would rather enter the data by hand onto a hard copy. This form can be used to report on more than 50 engineers by photocopying or printing multiple copies of the form, but users should be aware that a single copy of the form contains entry lines for only 50 engineers.
